Support Packages

This is developed in partnership with the young person and the key worker / social worker/young persons adviser (YPA)

1. Assessment and planning
Signs of Safety framework provides an evidence-based approach to our assessment, planning, intervention and reviews that is based on a genuine partnership with young people and the practitioners.

2. Life Skills
Focused support to increase resilience and self-esteem, better manage relationships and increase confidence building.

3. Healthy Eating
Increasing health outcomes through health surgeries; advice on how to keep fit and healthy, diet plans and substance awareness.  Support to access specialist support such as GP, Dentist, Community Mental Health teams, Substance Misuse services and Sexual Health Services.

4. Key work sessions
Using the Every Child Matters outcomes framework, we develop meaningful support and risk management plans to meet the holistic needs of young people and provide regular reports to the Social Worker.

5. Education
Support to access career sessions, mainstream education, apprenticeships, training and work ready programmes to secure sustainable employment opportunities.

6. Safeguarding
Our approach to safeguarding is underpinned by the key principles of the Children’s Act and the Care Act, with a greater emphasis on prevention.

7. Independent living skills programme
Support young people to carry out day to day activities such as cooking, paying bills, cleaning, shopping and basic home maintenance
